Job Summary
The ComPAC team provides patient care and supportive services directed towards achievement of positive patient care outcomes, maximizing the patient’s involvement in his/her own plan of care and reducing the length of hospitalization. The ComPAC nurse plays a critical role in patient engagement and patient experience, quality and safety, and cost-effective care.
Key Responsibilities
- Responsible for phone triage of symptom-based calls from patients.
- Assess the patient’s needs, clinical symptoms, follows symptom-based protocols and gives appropriate clinical advice per recognized guidelines.
- Manage and provide care to the patient and their family.
- Drive quality patient care provided by other members of the patient care team.
- Assess, plan, implement, and evaluate the nursing care of patients in alignment with the professional practice model.
- Retrieve outside medical records and communicate with the surgical and anesthesia providers to ensure optimization of surgical patients.
Requirements
- Bachelor’s Degree in nursing from a CCNE or ACEN accredited nursing program required.
- Four years of RN experience working in acute care or perioperative setting is strongly preferred.
- Current Ohio license is required.
- Maintains current Ohio RN license.
- Meets mandatory educational and health requirements.
- Demonstrates computer and job skills required to meet job functions.
